Puppy Adoption Application Form

Thank you for your interest in a Sierra Vista Labradoodles. The purpose of this application is to insure the best Owner/puppy match as possible. To best achieve this we ask you to be as complete as possible in your responses.

In order to reserve a puppy, this application form must be completed on-line. Once the application is reviewed and accepted, you will be notified and then a deposit (non-refundable) of $250.00 will required to secure your space on the waiting list.

This deposit will be applied to the total purchase price of the puppy. An application received without a deposit will not reserve a puppy. We reserve the right to decline any application. If your application is declined you will receive a full refund of your deposit.

20. We request that a dog be returned to us, at any point in time, should you find yourself unable to care for it. Are you willing to agree to this? (Yes/No)
